1. Nature of services

Auren Vitality is a concierge wellness service that coordinates access to peptide and related wellness protocols prescribed by licensed medical providers and dispensed by licensed compounding pharmacies. Auren Vitality is not a medical practice, hospital, or emergency service. Any clinical decision, prescription, or diagnosis is made solely by the licensed provider overseeing your care, not by Auren Vitality or its staff.

2. Investigational and off-label use

I understand that many peptide compounds are not approved by the U.S. Food and Drug Administration for the purposes for which they may be recommended, and that compounded preparations are not FDA-approved products. Their use may be considered off-label or investigational. Long-term safety data may be limited or absent. No representation has been made to me that any protocol is FDA-approved, proven effective, or free of risk.

3. Risks I accept

Potential risks include, without limitation: injection-site pain, bruising, swelling, or infection; allergic or hypersensitivity reactions, including rare anaphylaxis; nausea, vomiting, diarrhea, or appetite change; headache, fatigue, dizziness, or sleep disturbance; fluid retention, joint pain, or numbness and tingling; changes in blood glucose, blood pressure, heart rate, hormone levels, or thyroid function; interaction with medications, supplements, or existing conditions; theoretical stimulation of existing or undiagnosed malignancy; and outcomes that differ from, or fall short of, what I hoped for. I understand this list is not exhaustive and that unforeseen effects may occur.

4. No guarantee of results

No promise or guarantee has been made to me regarding any specific result, timeline, degree of improvement, or outcome. Individual response varies. Testimonials, case examples, and general information I have seen do not predict my own result.

5. My disclosures and responsibilities

I affirm that the information I provide, both here and to the licensed provider overseeing my care, is true and complete to the best of my knowledge. I will promptly disclose any change in my health, medications, supplements, pregnancy status, or intention to become pregnant. I will follow the instructions given to me for storage, dosing, and administration, and I will not alter, share, resell, or transfer any prescribed preparation. I will seek independent medical attention, or call emergency services, for any urgent or severe symptom.

6. Not a substitute for medical care

I understand these services do not replace evaluation and treatment by my primary care physician or any specialist, and that I should maintain an ongoing relationship with my own physician. Nothing provided to me by Auren Vitality constitutes emergency medical care.

7. Right to withdraw

I may decline any recommendation and may discontinue any protocol at any time. I have been given the opportunity to ask questions, and my questions have been answered to my satisfaction.

8. Release and waiver

To the fullest extent permitted by law, and in consideration of the services provided to me, I release and hold harmless Auren Vitality, together with its owners, officers, employees, contractors, and affiliates, from any claim, demand, liability, cost, or expense arising out of my participation in these services, including claims based on ordinary negligence. This release does not apply to gross negligence, willful misconduct, or any liability that may not lawfully be released, and it does not extend to the independent acts of any licensed medical provider or pharmacy.

9. Privacy and referral

I authorize Auren Vitality to share the information in this form with the licensed providers and pharmacies involved in coordinating my care, so that they may contact me directly to complete their own intake. My information will be handled in accordance with applicable privacy law and will not be sold.

10. General terms

This agreement is governed by the laws of the State of Texas, without regard to conflict-of-law rules. If any provision is found unenforceable, the remaining provisions stay in full force. This document is the entire agreement between us on this subject and may be modified only in writing.
